Ready to enroll? It's as easy as 1,2,3! Unlike some dental plan applications, the Delta Dental Patient Direct™ enrollment form is short and simple. You can enroll online right here or over the phone 1-800-455-0473 (toll free).
How do I pay for Delta Dental Patient Direct™? Delta Dental Patient Direct™ accepts VISA and Mastercard, as well as electronic funds transfers from checking or savings accounts. Credit cards take 24-hours to process. Bank drafts take 3-5 days. If enrolling on-line, simply return to the site, log-in, and access your enrollment materials. To keep the cost down, we do not accept mailed checks. You can pay online or over the phone by calling 1-800-455-0473 (toll free).
What will I receive after I pay? Once you’ve enrolled and paid your annual fee, you’ll receive a Membership Card and a Membership packet. If you enroll online, you can print these from your computer after 24 hours. Please be aware, you will be required to create a username and password when enrolling online and remember the username and password for future access to your membership information - Be sure to write down your username and password. If you enroll by phone, they will be mailed to you - usually within 7 - 10 days. If you are not completely satisfied with your savings and benefits from the Delta Dental Patient Direct™ program you will be given a complete refund of your membership fee. Refunds must be requested within 30 days of enrollment to the program.
